Patients must be dentally fit to proceed with your chosen dental plan. Whether a patient is deemed dentally fit for treatment will be at the clinician’s absolute discretion. Additional treatment required to make a patient dentally fit and suitable for your chosen treatment plan will be charged at an additional cost.

Appointments are subject to availability and are with Dr Ash Parmar & Dr Luk Athwal only. The 10% off cosmetic treatment plan discount is non-exchangeable, non-transferable, and is not redeemable for cash or other treatments. All sales are final. In the event that Dr Ash Parmar or Dr Luk Athwal are unavailable for any appointment scheduled in relation to this offer, Smile Design By Ash will use best efforts to rearrange the appointment.

Where the chosen treatment plan is interrupted as a result of an appointment, or multiple appointments, being cancelled as a result of Dr Ashish Parmar or Dr Luk Athwal being unavailable and cannot be completed due to continued unavailability, Smile Design By Ash will refund the patient the amount previously paid for the treatment. For clarity, where two appointments have been attended and paid for by the patient, and the final appointment must be cancelled by Smile Design by Ash, and cannot be rearranged, only the amount already paid for the final appointment will be refunded to the patient. 

The 10% off cosmetic treatment plan discount cannot be used in conjunction with any other offer. Smile Design By Ash accepts no responsibility for any damage, loss, liabilities, injury or disappointment incurred or suffered as a result of the use of this offer or receipt of the treatment. Smile Design By Ash does not guarantee that the treatment will not cause damage of any kind (whether direct or indirect) to the patient’s teeth, and therefore the practice accepts no responsibility in the event that the treatment causes any damage to the patient’s teeth.

Nothing in these terms and conditions excludes or limits liability for personal injury resulting from Smile Design By Ash negligence or for fraudulent misrepresentation, or for any other liability which cannot be limited by law. The offer is open to people aged 18 and over only. Smile Design By Ash can vary or replace these conditions from time to time, at its sole discretion and reserves the right to withdraw this offer at any time. If any provision or part-provision of these terms and conditions is or becomes invalid, illegal or unenforceable, it shall be deemed deleted, but that shall not affect the validity and enforceability of the rest of the terms and conditions.
